Welcome to Novi Veterinary Clinic. We look forward to meeting you and caring for your pet. Please complete the forms below for each pet you need to have seen. We must have all prior medical history with doctor’s notes, any previous labs, and vaccine records (even if overdue) submitted at the same time, or within 24 hours to complete the registration process. Please let us know if you do not have any medical/vaccine history on the pet. Once everything is received, we will call you to get scheduled.
For Family Puppy Store Clients: We will need all the Health Records Signed by a Vet, and the Rabies Certificate in applicable. These originals were given to you at the time of purchase. Our office does not keep any duplicates on file. Once everything is received, we will call you to get scheduled.
Please note: This process can take 2-3 business days, once we have all the required paperwork. we are currently scheduling new patient appointments 2-3 weeks out. If you have a sick or injured pet, please visit a veterinary emergency hospital closest to you.
